Job Purpose
To provide high-quality, person‑centred care and support to individuals in their own homes, enabling them to maintain independence, dignity, and quality of life.
Key Responsibilities
Personal Care
* Assist clients with personal hygiene, including washing, bathing, dressing, and toileting
* Support with mobility, transfers, and use of mobility aids
* Assist with continence care and skin care
Medication Support
* Prompt, assist, or administer medication in line with care plans and company policies
* Maintain accurate medication records
Daily Living Support
* Prepare meals and assist with feeding where required
* Support with light household tasks such as cleaning, laundry, and shopping
* Assist with appointments and social activities
Emotional & Social Support
* Provide companionship and emotional support
* Encourage independence and social interaction
* Promote mental wellbeing and dignity at all times
Care Planning & Record Keeping
* Follow individual care plans and risk assessments
* Maintain accurate and timely care records
* Report any changes in a client’s condition to the appropriate person
Health & Safety
* Follow safeguarding policies and procedures
* Adhere to infection prevention and control measures
* Report accidents, incidents, or concerns immediately
Communication & Teamwork
* Communicate effectively with clients, families, and healthcare professionals
* Work collaboratively with colleagues and management
* Attend training, supervision, and team meetings as required
