Sandwell MBC and South West Birmingham NHS Trust are working in partnership to provide short-term reablement in a newly built integrated Social Care and Health Centre, which opened in December 2022. The centre is an 80‑bed reablement centre based on Harvest Road, Rowley Regis.
We are recruiting for several roles within the organisation. This will be an exciting opportunity to work in a service supporting individuals to regain their skills and confidence so that they can return home.
This role offers the perfect opportunity to work as part of the management team for the new centre, providing leadership support during the night to staff and supporting individuals to maximise their independence and return to a community setting.
You must be genuinely driven with a desire to make a real difference in people's lives, an excellent communicator, a good team player, and passionate about empowering and supporting service users to achieve the required independence.
Duties
* Assist in the leadership of the unit, including the supervision and training of staff.
* Assist to reduce long‑term care needs by promoting independence.
* Manage staff and building during the night shift.
* Assist in reducing readmissions to the acute trusts.
* Work in integrated collaboration with partners.
* Promote service‑user well‑being and independence.
* Contribute to all aspects of the physical and emotional care needs of service users.
You will also support the nursing team in specific areas such as medication, assessments, care plans and care reviews.
Working Hours
The shift is 9:15 pm until 7:15 am and you will work on a 2‑week rolling rota which includes 3 nights one week and 2 the next.
