The post holder will be an Advanced Nurse practitioner who candemonstrate the following:-
* Assess, diagnose, plan, implement and evaluate treatment/interventionsand care for patients presenting with undifferentiated and undiagnosedconditions and complex needs.
* Clinically examine and assess patient needs from a physiological andpsychological perspective, and plan clinical care accordingly.
* Proactively identify, diagnose and manage treatment plans for patientsat risk of developing a long-term condition (as appropriate).
* Post registration experience in at least one of the following areas,relating to the service applying for: Out of Hours Urgent Primary care,General practice, walk in centre, A&E.
* Be able to see patients of all ages
* Willing to undertake predominantly face to face with telephoneconsultations when required.
* Ability to conduct remote consultations/assessments
* Preferred skill experience in Emis.
Main duties of the job
* Patient Assessment:Conducting thorough physical exams, taking medical histories, and ordering diagnostic tests.
* Diagnosis and Treatment:Making independent diagnoses, developing treatment plans, and prescribing medications within their scope.
* Chronic Disease Management:Managing chronic conditions like diabetes and COPD, working with patients to achieve optimal health.
* Acute Care:Providing care for acute presentations and minor health problems.
* Referrals:Making appropriate referrals to specialists or organizing emergency admissions when needed.
* Investigations:Ordering necessary blood tests, radiology, and other investigations.

b.Manage patients presentingwith a range of acute and chronic medical conditions, providing subject matterexpert advice

e.Provide advanced, specialistnursing care to patients as required in accordance with clinical based evidence.
f.Undertake the collection ofpathological specimens

h.Process and interpretpathology and other test results as required
i.Provide chronicdisease clinics, delivering patient care as necessary, referring patients tosecondary/specialist care as required
j.Maintain accurate clinical records inconjunction with extant legislation
k.Ensure SNOMED CT codes are usedeffectively
l.Maintain chronic disease registers
m.Chaperone patients where necessary
n.Prioritise health issues andintervene appropriately
o.Support the team in dealing withclinical emergencies
p.Recognise, assess and referpatients presenting with mental health needs
q.Be an extended andsupplementary prescriber, adhering to extant guidance
r.Support patients in the use of theirprescribed medicines or over the counter medicines (within own scope ofpractice), reviewing annually as required
s.Contribute to practice targets (QOF, etc.),complying with local and regional guidance
t.Liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients, etc.)
u.Delegate clinical responsibilitiesappropriately (ensuring safe practice and that the task is within the scope ofpractice of the individual)
v.Support the clinical team with allsafeguarding matters in accordance with local and national policies
w.Deliver opportunistic health promotion whereappropriate

* Experience of practice within the four pillars
* Job plan that demonstrates advanced nursing practice and has equity with peers working at this level

Qualifications
* Registered Nurse with Nursing and Midwifery Council
* Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
* Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
* Qualified Independent Nurse Prescriber on the NMC register
* Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
* Meets the standards for registered ANP working at advanced level
* Qualified triage nurse
* Teaching qualification
