Your role –
7. Practice Nurse with strong Chronic Disease Management experience
8. Mix of treatment room duties and CDM
9. Full time ideally but open to Part Time as well
10. Work across both practices – very close together
11. Flexible on days and hours to suit you
12. Work alongside the other nurses in a close-knit team
