Overview
Job Summary Support the practice-based Clinical Pharmacists to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes. The post holder will work as part of a multi-disciplinary team and support all practice and PCN staff to develop and improve processes. A key part of the role will be dealing with daily medication requests and queries and communicating with patients and Community Pharmacies. You will work at two/three different practices.

This is a varied role and is actually based across two practices, which will enable variation of work undertaken across them.
Responsibilities
* Manage incoming medication requests and queries from patients, pharmacies, care homes, etc.
* Assist the practice admin team with medication queries.
* Liaise with patients to resolve queries and support their use of medication.
* Contribute towards the improvement of prescribing processes in general practice.
* Promote electronic repeat dispensing and online ordering.
* Carry out housekeeping in patient records (e.g., dose optimisation and medicines synchronization).
* Use and maintain information systems and databases relevant to the position (incident reporting, medicines optimisation, prescribing data, cost savings and patient outcomes).
* Handle sensitive or confidential information in line with Data Protection Act, Caldicott Guidelines and the Confidentiality Code of Conduct.
* Utilise GP practice clinical systems to conduct prescribing audits.
* Undertake, participate in and share the outcomes of clinical audits to facilitate improvement and changes in practice.
* Support the role of the Clinical Pharmacist in delivering primary care funding mechanism indicators (DES, GP specification, QOF).
* Participate in practice meetings, patient participation groups, and other meetings to improve engagement and promote medicines optimisation.
* Plan and organise own workload to meet deadlines; undertake learning and development to maintain required knowledge and skills.
* Carry out other duties appropriate to the skills and competencies of the postholder as priorities change.
Qualifications
Essential
* Professional registration with GPhC
* CPPE completed
* BTEC/NVQ level 3 or equivalent in pharmaceutical sciences
* Evidence of continued professional development (CPD)
* Experience of working as a qualified, registered pharmacy technician in primary care
* Knowledge of GP clinical system: EMIS
* Ability to influence and persuade partners and stakeholders
* Understanding of NHS agenda, government targets and healthcare policy aims
* Knowledge of pharmacy law and ethics and current legislation; data protection and confidentiality
* Computer literacy and ability to use Microsoft Office
* Strong organizational and time-management skills with the ability to prioritise workload
* Analytical thinking and problem-solving abilities
* Excellent verbal and written communication; capable of adapting to the audience
* Team player with the ability to work independently
* Flexible, patient-focused approach; self-motivated and proactive
* Willingness to travel between sites as required
* Excellent time keeping and prioritisation skills
Desirable
* Advanced theoretical and practical knowledge of general practice and evidence-based medicine
* Understanding of healthcare policy implications for General Practice
* Influencing and negotiating skills
* Ability to analyse and interpret prescribing data
