The role
We’re looking for someone to work in our Hospital Discharge team. You’ll work closely with integrated discharge hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their own homes in the community.
Job Requirements
* You’ll complete tasks to help customers get home safely from hospital, such as property inspections, collecting belongings, dropping off food parcels, or arranging quotes for deep cleans to facilitate discharge.
* You’ll carry out holistic assessments with customers and use an outcome star to create an action plan to support their well-being at home.
* You’ll conduct ad hoc or regular visits to individuals in the community needing support, checking homes are warm and safe, medication and essentials are available.
* You will establish links within the voluntary and community sectors to support a holistic, tailored approach based on customer needs.
What We’re Looking For
* Excellent communication skills.
* Experience working with vulnerable and/or older people.
* Understanding of excellent customer service principles.
* Ability to work under pressure and manage daily tasks efficiently.
* Strong ICT and administrative skills.
* Experience managing high caseloads.
* Ability to identify issues to prevent discharge delays.
* Full UK driving license and own vehicle with business insurance.
* DBS check required.
