Role Overview
This role involves providing holistic assessment and treatment for patients with long‑term conditions and frailty within an urgent care setting. The post‑holder must work flexibly across traditional professional boundaries to ensure continuity of care for patients presenting with a range of conditions, including cardiovascular, respiratory, musculoskeletal, orthopaedic, and neurological issues.
Responsibilities
* Deliver high‑quality physiotherapy assessment and treatment to patients presenting acutely within the Emergency Department (ED), Clinical Decision Unit and Acute Medical Unit (AMU) as part of a Same Day Emergency Care (SDEC) model.
* Conduct rapid, holistic assessment and develop personalised care plans that enable early decision‑making to support safe discharge, admission avoidance, and improved patient flow.
* Work as part of a multidisciplinary frailty team, leading elements of service delivery and supporting junior staff within the frailty therapy team.
* Collaborate across professional boundaries to support a wide range of patients with complex needs.
* Operate flexibly across a seven‑day rota, including approximately one weekend shift per month.
* Ensure continuity of care for patients presenting with diverse health conditions, coordinating care and follow‑up as necessary.
