We are looking for dynamic, visionary and hard-working candidates to join our expanding Respiratory Department. We are advertising a full time post for excellent candidates to join the COPD Team. The team consists of Band 6 and 7 Respiratory CNS’s and Respiratory Specialists Physiotherapists. The team will be responsible for identifying and assessing inpatients in hospital. Suitable in-patients with an exacerbation of COPD who are from the Swansea Bay area will be supported at home by this team. The team also works with patients ,GPs and Virtual Wards to prevent avoidable admissions .

Main duties of the job

The successful candidates will be expected to work with colleagues to deliver a unified respiratory service across Swansea Bay UHB.

Wards are visited daily to identify and support patients who are suitable for an early discharge and patients are then followed up at home

The team manage a caseload of patients to provide education, teach self management skills and monitor recovery from acute exacerbations of COPD in patients homes

Education is encouraged in the team by formal education and an in house education programme.

The successful applicant will be supported via an agreed bespoke induction plan and at regular MDT meetings.

Qualifications

Essential criteria

1. Registered Nurse & current NMC Registration
2. Specialist post graduate qualification or equivalent experience
3. Proficient level of knowledge in clinical speciality Knowledge of service improvement
4. Able to undertake a literature search and critically appraise research papers

Desirable criteria

5. Teaching qualification (or equivalent experience
6. Advanced clinical skills Non-medical Prescribing

Experience

Essential criteria

7. Experience in the relevant specialty
8. Experience of working in a multiprofessional environment
9. Experience of undertaking audit

Desirable criteria

10. Experience of participating in research
11. Experience of participating in practice change

Skills and Abilities

12. Able to demonstrate effective leadership and management skills
13. Be able to demonstrate tact and diplomacy when working with others using good communication skills
