Main duties of the job
We offer a dynamic mix of chronic and acute work includingthe Portsmouth Clinical Assessment Service. There is also opportunity in other areas such as occupational health andsome very exciting research opportunities.
GPs at the practice work in small GP teams and holdtheir own personalised list. This gives unrivalled continuity for chronic carebut with the stability and efficiency of a larger practice.
Our Same Day Assessment Team is an advanced multidisciplinary team includingGPs working with nurse and paediatric practitioners. In addition to routine GP clinics we run ahighly organised nurse led chronic disease service. We have a fully developed network linked medicationteam including Clinical Pharmacists and Pharmacy Technicians providingsignificant savings in GP time. We also benefit from the Island City NetworkSocial Prescribing Team and a First Contact Physiotherapy team.

Our list size is 37,500 patients. We have a team of more than 100 staff which includes 8 GP partners, 10 salaried GPs, 1 nurse practitioner partner, 1 managing partner and multiple allied health practitioners.

SystmOne is used for patient notes. This is shared across community healthcare teams throughout the city improving communications and increasing patient safety.
We provide a Clinical Assessment Service similar to telephone triage for paramedics, A&E attendees and 111 callers eligible to be assessed by a GP. We can prescribe or book appointments for patients at their own surgery and if needed dispatch an ambulance.
We provide a wide range of in-house services including LARC, joint injections, occupational health, medicals and acupuncture.
We are an established training practice hosting FY2's, GP VTS, paramedic and nurse practitioner trainees as well as medical students with strong links to the new Portsmouth Medical School.
Job description
Job responsibilities
1. ln accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion;
2. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation;
3. Assessing the health care needs of patients with undifferentiated and undiagnosed problems;
4. Screening patients for disease risk factors and early signs of illness;
5. Developing care plans for health in consultation with patients and in line with current practice disease management protocols;
6. Providing counselling and health education;
7. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate;
8. Recording clear and contemporaneous consultation notes to agreed standards;
9. Collecting data for audit purposes;
10. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding handwritten prescriptions whenever possible);
11. Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate;
12. ln general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
