Cardiff Day Service is a specialist Day Service that provides support to adults with a learning disability who have complex support needs. People are supported by the service to access a wide range of valued activities and opportunities within the local community to meet their identified outcomes.
About the job
* Plan and provide high quality, person centred, outcome led support to people who use the service;
* Support and empower people who use the service to reach their full potential;
* Liaise effectively with a wide range of internal and external partners;
* Establish and maintain a range of community based integrated opportunities for people who use the service;
* Monitor and review services to ensure they remain person centred and that agreed outcomes are met;
* Effectively supervise support workers and actively contribute to the Personal Performance and Development Review process;
* Demonstrate a good understanding of and commitment to underpinning social care values and ethics;
* Contribute to the day to day running of the day service.
What We Are Looking For From You
* Experience of working with people with learning disabilities to establish community networks and resources.
* Experience of liaising effectively with a wide range of agencies and opportunity providers.
* To hold or have the ability to train and achieve the relevant QCF level required for the post.
* Flexibility and the ability to work in a dynamic and fast changing service.
* A commitment to offering positive person centred support to individuals using the service.
* A belief that people with learning disabilities have the right to achieve their full potential and have the right to be positively engaged within their community.
