Job responsibilities You will be supported to carry out the following duties, responsibilities and training To work with people, including their advocates and carers, to coordinate the identification of their needs, undertake comprehensive assessments of individuals including those with diverse or complex presentations to develop person-centred SMART Goals and support plans designed to meet needs in ways that maximises independence, choice and control, and are legal and affordable. To work as part of the multidisciplinary team, participating in the development of services and other staff, providing direction and assistance to support workers, other members of the team, and students. Take an active part in your own supervision and appraisal and to supervise and appraise allocated staff in accordance with the departmental supervision policy and councils appraisal policy. To be responsible for your continuous professional development, ensuring your practice is up-to-date and evidence based, attending all mandatory training and specialist training where appropriate. To work 37 hours Monday Friday.
