Nant Dowlais Health Centre is a new GP practice serving a 10,000 patient list in West Cwmbran. Led by a new, multidisciplinary partnership, we are seeking a Health Care Assistant with excellent clinical knowledge and experience to be a key member of our team, and also work alongside our senior clinical team to support them.

Main duties of the job
The post holder will work as part of a team that provides healthcare services to the patients of Nant Dowlais Health Centre, focusing on the direct needs of service users undergoing healthcare treatment.
The post holder is expected to work within clinical competencies with supporting base knowledge.
They will have completed a learning package at NVQ/QCF/CQFW level 3. They are expected to work within defined protocols, adhering to the Code of Conduct for Healthcare Support Workers in Wales under the direction, and close supervision of, and reporting to, a registered nurse.

Job responsibilities
Key Responsibilities
* Assist the Healthcare Team in the clinical assessment and management of patients presenting for healthcare needs within the practice.
* Undertake initial screening of new patients presenting at the practice under the indirect supervision of a registered nurse.
* Undertake health screening, testing, and clinical observations as required.
* Report all outcomes to the registered nurse.
* Recognize normal range parameters of clinical testing and report to the registered nurse when results are out of range, using agreed protocols.
* Carry out direct care duties under the delegation of a registered nurse, e.g., phlebotomy, observations, sample testing, spirometry, wound dressings.
* Rotate as necessary between the sites.
* Establish and maintain effective working relationships with internal and external agencies, interacting professionally with patients and colleagues.
* Implement care as planned and directed by the Registered Nurse, reporting any changes in the patient's condition promptly.
* Maintain written records of care in accordance with practice policy and the Code of Conduct for Healthcare Support Workers in Wales.
* Maintain IT systems, including inputting and extracting information.
* Promote health, prevent illness, and encourage self-care, respecting patient preferences.
* Act as a chaperone during clinical sessions as required, adhering to relevant guidelines/policies.

* Phlebotomy trained
