If so this role will provide you with a rare and exciting career opportunity to be part of our new service implementation.

Avon and Wiltshire Partnership (AWP) are looking for an exceptional person to provide leadership to deliver our vision of excellent care for people with a learning disability and autistic people supported by our specialist acute mental health inpatient service and outreach team.

AWP are working with SW NHSE to provide a new inpatient facility as part of the transformation of end-to-end pathways in the region. We aim to be a centre of excellence. The services are being designed and developed with clinicians and people with lived experience.

You will be instrumental in the ongoing development, implementation and delivery of the clinical and operational model for the outreach element of the service, delivering admission prevention strategies and supporting planned admissions and discharges.

We will support you to develop your clinical knowledge and skills and to further develop your operational and management leadership skills as part of the expanding expertise in learning disabilities and neurodiversity within AWP making this an exciting career opportunity.


Main duties of the job

To inspire, lead and evaluate a service of high quality, ensuring that people who have a Learning Disability and Autism are provided with a high-quality person-centred approach to care delivery that always considers people’s safety, privacy and dignity.

Work alongside the Advanced Clinical Practitioner and Matron to be a clinical and operational lead figurehead for The Kingfisher outreach service, utilising the best use of resources, ensuring the care and treatment activities delivered are safe, effective, caring and responsive to the needs and preferences of service users and carers. The role integrates aspects of clinical management, a small complex caseload and triage assessment, employee support, day to day operational leadership and service accountability.


Job responsibilities

Scope & Range

Lead and have operational responsibility for the clinical management of the Kingfisher outreach Service, ensuring mechanisms are in place to support, develop and monitor all aspects of performance,

Facilitate a learning environment that coaches and trains others to develop advanced clinical skills e.g. specialist assessment, planning, formulation and evaluation for individuals with a Learning Disability and/or Autism with a treatable mental illness

Provide an enhanced role and expertise which aims to prevent unnecessary acute psychiatric hospital admissions and (where safe, appropriate and in the persons best interest to do so) keep patients in their own homes, facilitating effective hospital discharge from The Kingfisher inpatient service.

Clinical Practice Leadership

Be a clinical Leader across the community environment, responsible for the operational management and accountable for the quality of all clinical activity within the designated area,

Hold a small caseload of highly complex service users for time limited interventions, assessment and triage

Confidential, sensitive and comprehensive, person-centred assessment of the full range of patient safety and wellbeing needs (including risk, behavioural and mental state assessments),

Work with the team to ensure that every service user has a SMART, person-centred, evidence-based and holistic care and treatment plan ensuring service users are supported to reach the optimum level of independence and that robust processes are in place to keep such plans under continual review,

Work with the team to ensure that service users can take an active role in the assessment management and evaluation of their care and treatment plan, as well as all relevant risk and personal safety plans,

Empowering service users to be the centre of their care, ensuring patient and carer participation in decision-making and promoting health and independence through awareness raising and care navigation to other professionals as appropriate,

Communication & Relationships

Work closely with key internal and external personnel including, social care, housing and non-statutory agencies.

Present information, some of which may be contentious, to staff groups, service user groups, carers groups and other stakeholders using a range of techniques and media.

To participate in and where necessary, chair clinical and core group meetings and to work collaboratively with other colleagues in the multi-professional team.

Provide a high standard of written and verbal communication as well as documentation that is clear and in line with Trust documentation standards

Communicate with individuals, carers and other visitors in a courteous and helpful manner, whilst being mindful that there may be barriers to understanding,


Person Specification


Education and Qualification

* Registered Health and Social Care Professional (e.g. Registered Nurse, Occupational Therapist, Allied Health Professional etc.)
* Certificated evidence of relevant Continuing Professional Development (CPD)
* Qualification in Management and/or leadership
* Advanced training in Positive Behaviour Support


Experience and Knowledge

* Substantial post-registration clinical practice, and experience of leading/supervising
* Significant demonstrable experience of leading teams in the effective delivery of care
* Experience of organisational change, transformation or cultural development work


Skills and Abilities

* Excellent written and verbal communication skills, including coherent data presentation and reporting
* Ability to assess, plan, implement, and evaluate programmes of care and treatment, including the component activities and tasks within such programmes


Other Requirements

* Driving license with access to a car
