The Bute Occupational Therapy Team is looking for a dynamic individual who is passionate about patient care, an influencer and possesses proven leadership experience along with excellent communication skills to lead and deliver high quality care. The OT Team Lead post will have professional and managerial responsibility of the Community OT team on Bute, as well as the HCSW’s who carry out reablement and other support tasks under the direction of the Community Nurses, AHP s and SW within the Multi-disciplinary Extended Community Team. The post holder will ensure the highest standard of personalised care is delivered to patients and their families in partnership with the Community Hospital, A&E, GP Practice, Adult Social Work and all members of the multi-disciplinary team on the Island. The Bute Extended Community Care Team is a busy team providing a wide variety of clinical care to patients in their own homes, as well as close links with the community hospital and GG&C Hospitals, to contribute to discharge planning. The Team work closely with our Social Work, OT and Physiotherapy colleagues to promote reablement and to provide a seamless service to our island community.
