An exciting opportunity has arisen for an experienced, enthusiastic and highly motivated Registered Learning Disabilities Nurse (RNLD) to join our Learning Disabilities Intensive Support Team (LDIST) in Swansea Bay University Health Board as a band 6 clinical nurse. LDIST is a service with 3 bases: Swansea, Cwm Taf and Cardiff & The Vale. This post is for LDIST Cwm Taf base.

Our standard operational hours are 8am to 8pm Monday to Friday and 9am to 5pm weekends and bank holidays; with occasional flexibility required to meet service needs and ensure continuity of care.

We are committed to delivering person-centred, evidence-based care focused on preventing unnecessary admissions to acute admission units, reducing placement breakdowns, and supporting the safe and timely discharge from inpatients.

We are seeking a dynamic and compassionate individual who is inspired by change and dedicated to providing high-quality community-based care. The post holder will work as part of a team providing specialist nursing input to adults with learning disabilities and additional health needs in conjunction with the wider multi-disciplinary team. Excellent communication skills, a positive approach, and a strong commitment to promoting dignity, respect and confidentiality are essential.

Main duties of the job

The post holder will provide specialist nursing care and clinical leadership within the team, supporting adults with learning disabilities who are experiencing a change in mental or physical ill health and associated challenging behaviours in the community. They will deliver person-centred, evidence-based interventions that promote health, wellbeing, and independence. Responsibilities include assessing health needs, developing and implementing care plans, and supporting safe and timely inpatient discharges while preventing avoidable admissions working in collaboration with the multi-disciplinary team. They will provide expert advice and support to patients, families, and carers. Act as a role model to provide supervision and mentorship promoting staff development and contribute to service development and quality improvement initiatives. They will deliver strong compassionate leadership to team members, have excellent communication and organisational skills, maintain accurate clinical and administrative records
This post is for the Cwm Taf base, however there is an expectation to cover any of the other bases in LDIST as needed. A full driving licence and access to a suitable vehicle for business purposes is needed for prompt travel between settings some of which may be rural, to visit patients in a variety of settings, includes lone working

Knowledge

Essential criteria

1. RNLD NMC registered. Diploma level qualification on relevant subject area. Degree or ability to work to Degree Level in a related field of study or equivalent experience.
2. Possess knowledge of relevant legislation e.g. Mental Health Act, Human Rights Act

4. experience/ knowledge of Risk Management.
