Job Title: Advanced Practitioner – Transitions Team
Location: Nottinghamshire
Rate: £43.53 per hour
We are seeking an experienced and dedicated Advanced Practitioner to join our Transitions Team based in Nottinghamshire.
Primary Responsibilities as an Advanced Practitioner:
* Provide high-quality support and guidance to young adults transitioning from children's services to adult services.
* Work collaboratively with multi-disciplinary teams to ensure a smooth transition process.
* Conduct assessments and develop personalized care plans.
* Monitor and review the effectiveness of care plans and make necessary adjustments.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK license
* Post-qualified experience
* Degree level or equivalent in Social Work
