Shift pattern: 18.5 hours per week (part‑time, Pro Rata salary). A live NMC UK PIN is required. This is a part‑time post for the Huntington’s Disease (HD) Nurse based in Castlemilk, Glasgow City HSCP, providing support to patients and carers across Glasgow City.
Huntington’s Disease Nurse Role
The post holder will take a lead role and contribute to the clinical management of people with HD, specifically in respect of diagnosis, symptom management, and treatment options. The role is embedded within the multidisciplinary team to ensure seamless, personalised care.
Responsibilities
* Optimize access and input from existing HSCP services, ensuring that there will be care coordination for a seamless and personalised service throughout the individual’s journey, linking with health, social care and voluntary services.
* Collaborate within the MDT, embedding HD support across neurology, psychiatry, clinical genetics, social work, community rehabilitation and other health and social care teams to enhance care coordination.
* Support family members and carers with advice and guidance around finances, future care planning, post‑diagnostic support and carer support.
* Maintain an “easy in, easy out” flow through services with access to the right service at the right time, minimising risks and avoiding crisis. Initiate and promote research, evidence‑based practice and monitoring of service delivery.
* Act as an expert information resource, providing advice, education and practical support for patients, their families and all involved professional staff.
Requirements
* Registered Nurse on the NMC Register with relevant experience within an acute or primary care setting.
* Proven experience in HD or other neurological conditions, demonstrated through direct support to this patient group, ability to tailor and adapt care plans, and multi‑agency working.
* Evidence of CPD in person‑centred care, psychological responses to stress and distress in dementia, palliative care, counselling skills or similar learning or experience.
