Learning Disabilities Social Worker
Location: Rotherham
Salary: £33 per hour

Position Overview: We are currently seeking a skilled and experienced Social Worker to join our Learning Disabilities Team on a temporary basis. As a Learning Disabilities Social Worker, you will play a crucial role in assessing needs, developing support plans, and coordinating services to enhance the quality of life for adults with learning disabilities.
Key Responsibilities:
1. Conducting comprehensive assessments of adults with learning disabilities to identify their strengths, needs, and goals.
2. Collaborating with individuals with learning disabilities, their families, and other professionals to develop person-centered support plans.
3. Providing ongoing support and advocacy to ensure access to healthcare, education, employment, housing, and social activities.
4. Coordinating with multi-disciplinary teams to deliver integrated and holistic support services.
5. Advocating for the rights and interests of individuals with learning disabilities, promoting self-determination and empowerment.
6. Monitoring and reviewing support plans, assessing progress, and making adjustments as necessary.
7. Maintaining accurate and up-to-date records of assessments, interventions, and outcomes.
Qualifications and Experience:
8. Qualified Social Worker with relevant experience in supporting adults with learning disabilities.
9. Registration with Social Work England or equivalent regulatory body.
10. Knowledge of relevant legislation, policies, and frameworks related to learning disabilities.
11. Strong assessment, communication, and interpersonal skills.
12. Ability to work collaboratively as part of a multi-disciplinary team.
13. Commitment to promoting the rights, dignity, and well-being of individuals with learning disabilities.
