Overview

An exciting post is now available for a Band 4 Therapy Assistant to work within multidisciplinary community settings and support delivery of high quality therapy interventions to patients within their own homes. This post supports ongoing care and management of clients within the community who are in receipt of Integrated Placement Of Care Hub (IPOCH) General Nursing Assistant (GNA) package of care. The role offers the opportunity to enhance skills within the community therapy services, linking in with the wider support network and being part of an integrated support system. You will work across our Discharge to Assess beds and across all care communities as required. It is expected that the location of this post will be within the IPOCH Transfer of Care Hub with an expectation of supporting colleagues across the CCICP footprint when needed.

The successful candidate will have an Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5) or equivalent. The Band 4 Therapy Assistant will manage a defined workload of patients who are currently receiving a care package from the GNA service, or reablement/block care provider following triage by the qualified therapist. The service focuses on a "home first" approach, rehabilitation and promotion of independence for patients leaving acute/bed-based settings or requiring step up care in their own homes. It also supports end of life patients in their own homes, providing a dignified and quality approach to care for the patients and their families at a challenging time.

The role will include independent assessment and formulation and progression of a person-centred rehabilitation plan. To work as part of the team providing a comprehensive rehabilitation service under the guidance of qualified staff for patients in their service. Good communication between the patient, families, IPOCH and therapy teams will be essential – providing realistic expectations of the service and up-to-date information on the current presentation of every patient on the caseload. The successful candidate will undertake record keeping using an electronic device for which training will be provided. Applicants must hold a valid UK driving licence.


Responsibilities

1. To independently complete specified holistic assessments, interventions and create rehabilitation programmes either independently or with help of a qualified clinician for community-based patients.
2. To follow departmental protocols and guidelines referring to the qualified clinician who is available for advice.
3. To hold a caseload as delegated by the qualified clinician, reporting back at agreed timescales and highlighting any necessary actions. Working independently and supporting the band 3 Therapy Assistant as required.
4. To work with an individual in a variety of settings, such as their home or 24-hour care facility, ensuring patient safety and dignity is always maintained.
5. To modify interventions and programmes within defined parameters and within their scope of practice.
6. Complete initial assessments and commission a range of basic equipment and adaptations as appropriate to the service user. Complete necessary risk assessments before prescribing basic equipment and review them on a timely basis.
7. To be responsible for the ordering and stock control of materials and equipment as required for the unit.
8. Instruct individuals, carers and other professionals in the safe use and maintenance of equipment and simple aids (e.g., walking frames, sticks, elbow crutches).
9. To refer patients to appropriate services and health care professionals where patients are found to have a complex health need.
10. Discharge patients within your workload after consultation with a qualified member of the team.
11. Attend multidisciplinary meetings and participate in clinical decision-making processes for individual patients as delegated by the qualified staff.
12. To ensure accurate, up-to-date clinical records are kept, with security and confidentiality maintained.
13. Work within their competency and seek qualified therapist guidance where appropriate.
