The salary for this permanent Adult Social Worker job is up to £45,091per annum. Main duties: Conduct statutory assessments and develop support plans for adults with mental health needs and autism. Collaborate across health and social care professionals to ensure integrated delivery of care. Monitor, review, and adapt care plans based on evolving needs. Provide crisis intervention support when needed. Advocate for service users rights, access to services, and personal goals.
