Qualified Social Worker- Community Mental Health
Are you a qualified and passionate Social Worker with experience in mental health? This is a fantastic locum opportunity to join a dynamic Older People's Community Mental Health Team based in Trafford, supporting adults with complex and enduring mental health needs.
You'll play a key role in providing holistic, person-centred support that empowers service users to live safely and independently in the community.

Responsibilities:
* Manage a caseload of older adults with complex mental health needs
* Complete assessments, care planning, and risk management
* Act as Care Coordinator under the CPA framework where required
* Support discharge planning and community reintegration
* Work in partnership with families, carers, and health professionals
* Contribute to safeguarding, supervision, and service development
To be considered for the role, ideally you have:
* Diploma/Degree in Social Work
* Social Work England registration
* 1+ years experience in a community mental health or similar adult social care setting
* Strong understanding of the Mental Health Act, Mental Capacity Act, and Care Act
* Confident working in a multi-agency environment and managing complex caseloads
