We are seeking a highly motivated individual to work collaboratively within a primary care setting, delivering outstanding care to a diverse patient population.

Key Responsibilities
As an Advanced Clinical Practitioner, you will be responsible for:
* Independently assessing, diagnosing, planning, implementing, and evaluating treatment and interventions for patients with undifferentiated and undiagnosed conditions.
* Providing expert clinical care for patients presenting with acute illnesses, long-term conditions, and complex health needs.
* Working autonomously to see patients in clinics, undertaking home visits where necessary, and managing a varied caseload.
* Demonstrating advanced clinical decision-making, assessment, and treatment skills.
* Acting as an independent prescriber, ensuring the safe and effective use of medications.
* Contributing to the development and implementation of new models of care and clinical pathways.
* Playing a key role in clinical governance, audit, and quality improvement activities within the practice.
* Mentoring and supporting the professional development of other members of the clinical team.
About You
The successful candidate will possess:
* A Master's degree (MSc) in Advanced Clinical Practice or equivalent.
* Current registration with the NMC or HCPC.
* An independent prescribing qualification (V300).
* Significant post-registration experience in a primary care, urgent care, or acute setting.
* A proven ability to work autonomously and make complex clinical decisions.
* Excellent communication, interpersonal, and organisational skills.
* A commitment to continuous professional development and evidence-based practice.
* Experience with the EMIS clinical system is desirable but not essential.
