56 Dean Street is an award‑winning HIV and sexual health service based in Soho with a particular emphasis on HIV prevention and the sexual health care needs of LGBTQI+ people. It is Europe’s busiest sexual health clinic with an attendance of approximately 110,000 patients per year. It cares for the third largest HIV cohort in the country with approximately 4,600 patients. 56 Dean Street has been commissioned to provide TransPlus, the first integrated gender, HIV and sexual health service by NHS England. Our vision is for an innovative and patient‑centred service, based on the principle that patients must have equal access to services whatever their gender identity with reasonable adjustments to the delivery of care to match the individual’s needs and circumstances.
The post is for a 10‑person Associate Consultant in Gender Health, HIV Medicine and Sexual Health within the TransPlus gender dysphoria service, for a 12‑month fixed term. The job is based at 56 Dean Street, Soho.
Main duties of the job
* Provide assessment and diagnosis of gender dysphoria as a lead clinician.
* Formulate treatment plans and provide endorsement for gender-affirming surgery where appropriate.
* Work closely with a multidisciplinary team of nurses, pharmacists and resident doctors to provide holistic and safe care.
* Attend and contribute to multidisciplinary meetings.
* Participate in research and audit within TransPlus.
* Provide advice, screening, diagnosis and treatment of patients with STIs, including HIV infection.
* Develop integrated HIV & sexual health services, optimise cost efficiency, implement new technologies and develop new initiatives.
Job responsibilities
* Provide assessment and diagnosis of gender dysphoria.
* Formulate and monitor treatment plans for patients diagnosed with gender dysphoria at TransPlus.
* Assess readiness of patients for gender‑affirming surgeries and refer where appropriate.
* Provide advice, screening, diagnosis and treatment of patients with STIs, including HIV infection.
* Participate in the local HIV multidisciplinary meeting at 56 Dean Street.
* Contribute to implementation of clinical pathways.
* Provide consultant advice and support to multidisciplinary team working in Gender Health, HIV and GUM clinics. Review complex cases requiring senior input.
* Manage own caseload of HIV patients.
* Provide supervision and support for nursing colleagues and junior medical colleagues providing management of patients with gender dysphoria, stable patient cohorts, management of seroconversion, starting/switching treatment and management of complex or unwell patients.
* Work with colleagues to modernise the HIV care pathway to ensure effective use of resources.
Some roles may require weekend shifts at multiple sites. Employment is subject to a six‑month probation.

Education and qualifications
* Full GMC Registration
* Relevant CCT or equivalent (equivalence must be confirmed by PMET/GMC by date if Advisory Appointments Committee (AAC))
* Entry on the General Medical Council (GMC) Specialist Register via one of the following: Certificate of Completion of Training (CCT) (the proposed CCT date must be within 6 months of the interview) / Certificate of Eligibility for Specialist Registration (CESR) / European Community Rights
* MRCP (or equivalent)
* Dip HIV and DIP GUM
* RCP Professional Certificate in Adult GIH
* DFSRH
* LoC SDI
* Higher research degree (MD or PhD)
* Teaching Qualification
* Training in Management
Experience
* Evidence of broad based training experience and competence in sexual health, contraception and HIV care equivalent to that required for a UK CCT
* Experience of audit, quality improvement and other aspects of clinical governance
* Clinical and educational supervision experience
* Equality and diversity training
* Specialist interest in sexual health, contraception or HIV Medicine
* Experience of setting up and conducting clinical research studies and collaborating with other researchers
* Experience of setting up and evaluating new sexual health or outreach services
* Experience of developing patient centred healthcare, especially for individuals with increased vulnerability and/or complexity
* Training in and practical experience of safeguarding
* Experience of working with colleagues, including commissioners at a regional or national level to help develop services or the specialty itself
