Exciting opportunities have arisen for suitably qualified individuals to take a lead on the day to day operational management of Community Learning Disability teams across the SBUHB or CTMUHB Directorate to support the Service Group to deliver key targets and priorities work collaboratively with key stakeholders to deliver quality specialist community based Learning Disability Services support the Directorate Manager in delivering the modernisation agenda for Learning Disabilities Services plan and deliver services and respond to operational pressures to ensure continuity of effective community service provision uphold and continue to facilitate a values based culture for Learning Disabilities services.
This post holders will be pivotal in developing further collaboration and liaison with key stakeholders to include commissioners and local authority partners to enhance person centred and co-produced delivery of care in line with strategic vision across community learning disability services.
The service is currently undergoing an exciting period of modernisation and as a result, change management skills will be key to enable the post holder to successfully deliver key changes to the community services model.

Main duties of the job
Community Learning Disability Teams provide specialist health care, advice and treatment to adults with a learning disability through locally based multidisciplinary care. The post holder is responsible for the day to day operational management of the Community Learning Disability Team to ensure the Service Group delivers on its key targets & priorities in a safe,sustainable and effective environment.The post holder is required to work collaboratively with the Clinical Lead Nurse and therapy Clinical leads for the delivery of quality specialist Learning Disability Services.This post supports the Directorate Manager and assists in the modernisation, planning and delivery of services within the Learning Disability Division.The post holder will act on own initiative to resolve capacity issues liaising with health professionals, consultants and other departments to ensure current and future targets are met.The post holder is central to creating and modelling a values based culture for LD services and managing delivery of person centred services in a seamless way.
