Role Description
We are seeking a dedicated Assistant Care Manager for a full-time, on-site position in Blackpool. This role involves supporting care management activities, including coordinating services for clients, assisting with case management, and supervising care staff. The Assistant Care Manager will work closely with clients to ensure their needs are being met, provide support for their well-being, and maintain accurate documentation. The role also includes delivering training sessions, liaising with social services, providing support for elder care, and contributing to care planning and delivery.
Qualifications
Experience in Case Management and familiarity with care coordination processes
Knowledge of Elder Care practices and Social Services
Background in Medicine or related healthcare fields
Proficiency in delivering Training related to care processes and client support
Strong interpersonal, organizational, and communication skills
Ability to work collaboratively as part of a team while maintaining individual accountability
Relevant certifications or qualifications in care management or health services are advantageous
