Occupational Therapist Salary - £44,075 - £49,282 (Grade 7) Hours - 37 Contract type - 1 full time, permanent Work Style - Field: working in communities across the city Wolverhampton City Council has new and exciting opportunities within our Occupational Therapy Service. We’re expanding and looking for experienced, motivated practitioners who want to lead practice, develop others, and make a tangible impact across our community. About the opportunity As a Community Occupational Therapist, you will work across the Discharge to Assess, Reablement and Community OT pathways to support people to return home safely, regain independence and remain well in their own environments. You will carry out proportionate functional assessments, provide timely rehabilitation, and identify equipment and environmental solutions that promote confidence and safety. You will work closely with care agencies, reablement teams, hospital discharge colleagues and community partners to ensure people receive the right support at the right time. This includes supporting care staff to implement safe and effective strategies, reviewing care arrangements, and helping to prevent, reduce or delay the need for ongoing support in line with the Care Act’s wellbeing and prevention duties. Your practice will be strengths‑based, person‑centred and focused on enabling people to achieve their outcomes in a way that supports independence, dignity and participation. What you’ll do Complete proportionate functional assessments to support safe and timely discharge from hospital under the Discharge to Assess model. Work directly in people’s homes to assess mobility, daily living skills, safety, and environmental barriers. Provide rehabilitation and reablement programmes, focusing on recovery, confidence, strength, and independence. Prescribe equipment and environmental solutions that support safe occupation and reduce care needs. Work closely with care agencies to ensure care plans are safe, appropriate, and aligned with OT recommendations. Review existing care packages and advise where care can be reduced, adjusted, or stepped down in light of improved functional ability. Support reablement teams by offering clinical guidance on safe techniques, goal setting, and monitoring progress. Identify and respond to urgent risks, such as falls, pressure areas, moving and handling needs, or unsafe environments. Develop strength‑based, person‑centred support plans that promote independence and reduce long‑term reliance on services. Liaise with hospital discharge teams, social care practitioners, equipment services, and community partners to ensure seamless transitions. Record assessments, decisions, and outcomes clearly in line with professional standards. Promote Care Act prevention and wellbeing duties by helping people stay independent for longer. Contribute to service improvement by identifying themes, delays, barriers, or opportunities across D2A, Reablement and Community OT pathways. What you’ll bring HCPC registration and substantial post‑qualification experience in adult social care. Proven ability to manage complex risk, apply clinical reasoning, and achieve outcomes that matter to people and carers.
