We are seeking an enthusiastic, motivated, and forward-thinking Occupational Therapist to join our Mental Health Hospital Discharge Countywide team on a fixed-term 12-month contract. You will work with working-age adults (18-70) and older adults (70+) and will be supported by a caring team.
Your role involves supporting effective and timely hospital discharges across East Sussex, collaborating with hospitals, the Sussex Partnership Foundation Trust, and voluntary sector partners to ensure safe and creative discharges centered on the adult's needs.

* Qualified and registered Occupational Therapist
* Ability to build positive relationships with clients, families, and professionals
* Self-motivated and committed to personal development
* Willing to meet travel requirements of the role
