The successful post holder will work across the Trust and be an integral part of supporting the Trust Dementia and Delirium Strategy. This is an exciting opportunity to improve the lives, care and experience of those living with dementia or delirium during their inpatient admission.
Key Responsibilities
* Assess patients with suspected or confirmed delirium during their inpatient stay and support best practice for the prevention and management of delirium.
* Provide formal and informal training to staff across the Trust on dementia and delirium best practices.
* Collaborate with the Cognitive Neurology Service to deliver nurse‑led outpatient clinics.
* Contribute to the care of people with dementia and delirium using services at St George’s Healthcare NHS Trust.
* Ensure awareness of the Dementia Quality Improvement Plan, Dementia Strategy, St George’s dementia and delirium innovations, KPIs and quality standards across the Trust.
Qualifications
* Experience of working across patient groups and disciplines with a strong enthusiasm for influencing the care of people with dementia.
* Excellent communication skills and confidence working across professional boundaries.
* Knowledge of dementia and delirium care pathways and quality standards.
* Previous experience in training or education roles is desirable.
