Overview
The Advanced Practitioner - Clinical Pharmacist plays a vital role in improving patient care by providing expert medication management, supporting clinical decision-making, developing the service and collaborating with other healthcare professionals to enhance the quality of care within a Primary Care Network. Focusing on medication safety and optimizing therapeutic outcomes, the role supports PCN goals of improving patient care by developing patient-centred care pathways, reducing hospital admissions, and enhancing overall healthcare delivery within the community.
Our team consists of 12 Clinical Pharmacists and 6 Pharmacy Technicians; Rugby Health PCN is seeking a second Advanced Practitioner - Clinical Pharmacist. With at least 2 years PCN experience and an Independent Prescriber qualification, the postholder will work with the AP-CP Lead as part of the wider Clinical Pharmacy Team.
Responsibilities
* Assess and triage patients, provide definitive treatment (including prescribing following policy, patient group directives, NICE guidelines, and local care pathways) or refer to other primary care team members.
* Manage undifferentiated conditions, identify red flags and underlying serious pathology, and take appropriate action.
* Use complex decision making to inform diagnosis, investigations, and management across a broad scope of practice.
* Apply personalised and population-centred care to enable shared decision making with patients.
* Complete relevant training to provide multi-professional AP clinical practice and CPD supervision across four pillars to other roles in primary care.
* Lead and line-manage the PCN Clinical Pharmacists located across practices within the Rugby Health PCN.
* Provide mentorship, support service development, and oversee the PCN Clinical Pharmacy service.
* Monitor performance of Rugby Health Clinical Pharmacists and support the Senior Pharmacy Technician as required.
* Collaborate with the AP CP Lead to manage and develop the service and team, providing cover for leave as needed.
* These duties are in addition to those of a PCN Clinical Pharmacist.
Qualifications
Essential
* GPHC Registration as a Clinical Pharmacist
* Master's degree in pharmacy (MPharm) or equivalent
* Specialist knowledge via postgraduate diploma level or equivalent
* Completed CPPE pathway
* Independent prescriber
* Enrol on or have completed the CP Advanced Practice Programme/Portfolio
* Ability to practice independently within the role
Desirable
* Member of the Royal Pharmaceutical Society
* Fellow of the Royal Pharmaceutical Society
Experience
Essential
* Minimum of 2 years post-qualification experience
* Experience supervising a clinical team
* Experience in leadership roles
* Experience with patients in a community setting
* Experience managing common acute and long-term conditions
* Ability to work within a team and autonomously
* Excellent written and verbal communication skills
* Ability to communicate complex information to staff, managers, and patients
* Ability to plan and review general medicine optimisation for long-term conditions
* Ability to work under pressure and meet deadlines
* Ability to prioritise and identify deviations from normal patterns
* Produce timely reports and influence others to comply with recommendations
* Adherence to legal, ethical, professional and organisational policies
* Accountability for delivering professional and direct service provision
* Ability to recognize limitations and refer to appropriate colleagues
* Maintain confidentiality and discretion
* Experience working across agency boundaries and in local partnerships
* Therapeutic and clinical knowledge of evidence-based healthcare
* Understanding of general practices and primary care prescribing concepts
* Excellent IT skills with GP systems (EMIS, AccuRx)
