Job Summary
This is a great opportunity for a registered Paramedic/Nurse practitioner or a trainee advanced care practitioner who is currently undertaking Advanced Care Practitioner training. You will work in a friendly, multi-disciplinary team, working alongside GPs assessing and triaging patients in the minor illness clinics and conducting home visits. This will include same day triage and as appropriate provide a definite treatment or make necessary referrals to other members of the primary care team.
Duties
* Act as a key contact for Care Homes and patients on the frailty register, offering proactive care.
* Advise patients on general healthcare, including self-management and signposting.
* Undertake the collection of pathological specimens.
* Maintain accurate patient records and documentation in compliance with legal and ethical standards.
* Support the team with clinical emergencies.
* Liaise with external services/agencies to ensure patients are supported appropriately (vulnerable patients etc).
Qualifications / Experience
* Educated to Degree Level in Paramedicine/Nursing or educated at Masters Level in Clinical Practice or equivalent experience.
* Active Registration with the Health and Care Professional Council as a Paramedic or the Nursing and Midwifery Council as an Advanced Nurse Practitioner.
* Experience administering injections and performing physical examinations is essential.
* Excellent triage skills with the ability to assess patient needs effectively.
* Demonstrated ability to provide exceptional patient care with compassion and professionalism.
* Strong communication skills, both verbal and written, to interact effectively with patients and colleagues.
