What You Will Do
* Work collaboratively with individuals, carers, and partner agencies to deliver integrated and strengths-based assessments
* Develop person-centred care and support plans that promote independence and successful outcomes
* Manage a diverse caseload and ensure timely responses to emerging needs
* Provide accessible information and immediate support services where required
* Assess risks to independence using a range of tools and contribute to risk management planning
* Support individuals through safe and effective hospital discharge planning
* Provide advice on funding options to maximise informed personal choice
* Protect individuals’ rights under the Human Rights Act and Mental Capacity Act, ensuring least restrictive approaches
* Complete financial assessments in line with statutory requirements and maintain accurate financial records
* Work alongside a range of providers and organisations to develop creative community-based solutions
* Prepare detailed reports for case conferences and safeguarding forums
* Contribute to investigations under Protection of Vulnerable Adults procedures
