Title: Community Nurse
Location: Gloucester
Salary: £22 - £30 P/H
The Role
1. Conduct health assessments and screenings within the community
2. Provide healthcare education and promote health awareness to individuals and groups
3. Administer vaccinations and immunisations
4. Monitor and manage chronic conditions, such as diabetes and hypertension
5. Offer prenatal and postnatal care to expectant mothers and new mothers
6. Conduct home visits to assess living conditions and provide care as needed
7. Collaborate with local healthcare providers and organisations to coordinate care
8. Advocate for patients' healthcare needs and access to resources
9. Provide wound care, medication administration, and basic treatments
10. Offer support and counselling to patients and their families
11. Keep accurate patient records and documentation
12. Respond to emergencies within the community
