The post-holder will work as a senior community nurse, co-leading the Primary Care Workstream within a large multidisciplinary community learning disability team in North Tyneside.
The post-holder will complete comprehensive assessments and interventions to improve the quality of lives and outcomes for patients.
The post-holder will carry a clinical caseload and have supervisory and managerial responsibilities for staff.
The post-holder will contribute to service development and audits.
The post-holder will develop and deliver training.
Primary Care Team Leadership
Co-lead the Primary Care Team within CLDT. The team focuses on supporting the physical health needs of adults with learning disabilities, working in health promotion to reduce health inequalities and improve access to primary and acute healthcare services.
The team supports patients with epilepsy, dementia, and other health conditions, helping the wider CLDT understand and incorporate physical health needs into assessments.
The Post-holder will manage a patient caseload, including individuals with complex mental health or behavioral needs, supported by specialist forensic, mental health, and positive behavior workstreams within CLDT.
Training and Supervision
The post will involve developing and delivering in-house and provider training to enhance workforce and carer skills in health promotion.
The post-holder will have line management and supervisory responsibilities for the primary care workstream, including clinical supervision and appraisals.
As a senior nurse, the post-holder will provide support and guidance to the wider CLDT team.
Service Development
Contribute to service development and audit activities for the wider CLDT.
