Overview
ARC Hounslow supports people over the age of 18, residing in the borough of Hounslow, who want to break a cycle of addiction to substances or long-term alcohol addiction. We offer a variety of psychosocial and clinical interventions to support clients throughout their treatment journey.
This role will hold a caseload of individuals and specifically provide support to individuals through offering 1-1 and group work interventions. The post holder will use their specialist knowledge and skills in substance misuse to undertake assessment, care planning, implementing and evaluating the case and treatment of clients. They will perform duty assessments to clients that are new to the service, in addition to acting as keyworker to a defined allocated caseload of clients, under the supervision of the designated manager within the team.
Responsibilities
* Hold a caseload and provide 1-1 and group work interventions to clients.
* Use specialist knowledge and skills in substance misuse to undertake assessment, care planning, implementation and evaluation of case and treatment.
* Perform duty assessments for clients who are new to the service and act as keyworker to an allocated caseload under supervision.
* Ensure delivery of care and management for the client group is compliant with expected standards and adheres to policies and procedures.
* Deliver evidence-based interventions including harm reduction approaches, motivational interviewing, relapse prevention, care planning, health education, detoxification programmes, mindfulness, counselling and group work in 1-1 and group settings.
* Continuously assess the psychological, emotional, social and rehabilitative needs of clients, considering family and community needs, and develop and evaluate care plans.
* Assess and case manage clients who inject drugs, including assessing injection sites and applying harm reduction principles; take a predominant role in duty assessments for new clients.
* Ensure compliance with Outcome Monitoring Tools and other quality measures to support contracted service delivery.
* Collaborate with internal and external colleagues to promote best practice, evidence-based services and recovery-oriented impact; lead on an area of specialism where appropriate.
* Manage the transfer of individuals between agencies and services.
* Promote comprehensive risk assessments and risk management across the service under supervision.
* Ensure understanding, compliance with and implementation of clinical governance.
