As a hospice liaison, you will be responsible for coordinating patient referrals, establishing relationships with local hospitals to secure referrals, and providing education for the community and referral sources on hospice care. You’ll make a meaningful difference by working closely with patients and their care team to ensure continuity of care, proper communication, and correct documentation.

Essential Functions:
The Hospice Liaison is responsible for generating new hospice business, maintaining existing services, and coordinating care for referred patients. The role involves educating the community and medical professionals about hospice services, and daily interaction with patients, medical staff, and referral sources to ensure continuity of care and proper communication. The position requires regular off-site engagement.
Qualifications:
* Passion for serving patients and families
* Education: Minimum of an Associate Degree or relevant work experience in a health-related field
* Experience: 2 years in clinical sales, hospice preferred
* Requirements: Reliable transportation, ability to sit, stand, bend, lift at least 25 lbs
