Stroke Liaison Nurse, Band 6 (11.5 hrs & 19.5 hrs posts)
An opportunity has arisen within the Borders Stroke Unit in Borders General Hospital for a flexible, enthusiastic individuals to assist in the provision of patient centred care.
The practitioners will have/develop high levels of decision making and clinical judgment, be able to work independently and in conjunction with other health professionals. You will assess and initiate treatment, investigations, advice, and management of patients using professional decision-making processes to ensure safe, patient centred care.
This will be facilitated in partnership with patients, carers, and families. You will provide specialist advice to healthcare professionals, other agencies, carers, clients, and relatives.
You will play a vital role as the Stroke Outreach Nurse, responding to Thrombolysis and thrombectomy calls. There will be an element of aftercare including running regular stroke review clinics.
They will assist the service to improve practice and work collaboratively across healthcare boundaries, implementing and evaluating evidence-based standards, guidelines and policies.
There will be a requirement for weekend working.
