<div><p>Brinsworth & Whsiton Medical Centre are offering an exciting opportunity for an enthusiastic nurse to join our practice team. This is a permanent, part‑time position for up to 25 hours, following the retirement of one of our nurses. The successful candidate will work three days per week.</p><h3>Main duties of the job</h3><p>The role needs to be worked flexibly in line with the practice opening hours (these may change).</p><p>Will work collaboratively within the general practice team to meet the needs of patients, support the delivery of the Quality & Outcomes Framework (QoF) and any other performance monitoring systems.</p><p>You may be requested to undertake duties/tasks that have not been identified in this job description; these will be discussed prior to undertaking them.</p><h3>Job responsibilities</h3><p>Assist in and perform routine tasks relating to patient care. Undertake and promote cervical cytology. Provide a comprehensive family planning service and sexual health service. Undertake spirometry and have undertaken relevant training. Manage newly diagnosed patients with asthma, COPD & diabetes.</p><ul><li>Undertake chronic disease management and prevention: Asthma, COPD, Diabetes, Hypertension, Hypothyroidism, IHD, Stroke, LD.</li><li>Perform wound care using aseptic technique and general wound care.</li><li>Undertake opportunistic screening of BP checks, height, weight, BMI, urinalysis, glucose monitoring and smoking status.</li><li>Administer injections under an individual prescription or patient group direction.</li><li>Offer a holistic approach to travel health including comprehensive advice for patients prior to travel.</li><li>Undertake ear syringing.</li><li>Competent in anaphylaxis and resuscitation techniques.</li><li>Child health and child protection, including immunisations.</li><li>Promote health and well‑being, through the use of health promotion, health education, screening and therapeutic communication skills.</li><li>Refer and use social prescribing teams.</li><li>Tend to home visits.</li><li>Clearly communicate the health status of the patient using appropriate terminology, technology and format.</li><li>INR monitoring if agreed by a shared care protocol.</li><li>Undertake infection control measures, including audits and annual reports.</li></ul><h3>Qualifications</h3><ul><li>Nursing & Midwifery – NMC registered.</li><li>Qualified to undertake cytology.</li></ul><h3>Experience</h3><ul><li>Experience of working as a practice nurse.</li><li>Previously worked in a general practice setting undertaking long‑term conditions such as diabetes, COPD, asthma, etc.</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p></div>
