Our Constitution Street service delivers a high-quality Care at Home and Housing Support service for adults with severe and enduring mental health challenges. Supporting 11 individual tenancies and community- based outreach support, the service operates on a 24/7 basis.
With the aim of equipping people with the skills needed to live independently and meaningfully within the local community, a strengths-based coaching approach to recovery is used to build confidence, maintain motivation and promote resilience. Alongside this we also provide practical support with food preparation, medication, budgeting and other daily living skills.
