Job Description
We're recruiting an Outreach Family Support Worker to deliver high quality, home based Early Help support to children, young people and their families. You'll hold a caseload, provide targeted interventions, and coordinate multi agency support using a whole family, strengths based approach.
The role focuses on preventing escalation to statutory services and improving outcomes for children aged where SEND applies).
Key Responsibilities
* Hold and manage a caseload of children and families with multiple or complex needs.
* Complete Family Help assessments and lead Team Around the Family (TAF) meetings.
* Provide intensive, targeted support to vulnerable children and young people, including those with emotional or mental health needs.
* Develop and coordinate multi agency plans that reduce risk and improve outcomes.
* Maintain accurate, timely case records, assessments, plans and chronologies.
Key Requirements
* Experience working with children, young people and families with complex needs.
* Strong assessment, planning and case holding skills.
* Ability to lead multi agency meetings and coordinate support.
* Excellent communication, organisation and report writing abilities.
* Knowledge of safeguarding legislation and Early Help frameworks.
Contract Length: 3 months initially with the view of an extension.Pay Rate: £15.87 - £20.87 per hourWorking pattern - 37 hours per week, Hybrid working arrangements.
