We currently have a number of exciting opportunities for the appointment of dynamic, self-motivated Band 6 Sisters/Charge Nurses to work as part of the Patient Flow Coordinator Team within Acute Medicine based at St Jamess University Hospital Leeds.
We are looking for enthusiastic individuals to take responsibility and actively manage patient flows across the Clinical Service Unit by implementing and maintaining processes that safely expedite patient placement and discharge.
The jobholder is responsible for the timely and appropriate placement of patients requiring admission to the Trusts bed base and for the escalation procedure from the Emergency Department.
You will develop advanced skills in identifying risks and solving complex issues, and gain valuable experience in balancing staff availability and skills against the needs of patients across the service.

Provide a visible clinical lead and role model to the nursing and wider team by delivering high quality compassionate care as an expert practitioner
Ensure that documentation standards are maintained in accordance with NMC and LTHT guidelines, whether written or computerised.
Help to set, monitor and seek to continuously improve standards and the quality of patient care, including those standards defined in local and national guidance
Work with Senior Sister / Charge Nurse to ensure that clinical governance processes at ward/dept level encompass measures to ensure the quality of the fundamental and essential aspects of care
Balance clinical risk and clinical decision making against context
Work with the Infection Prevention and Control Team and Facilities to monitor the environment to maintain and raise standards
