The Community Therapy Service team in Wallingford is looking for an enthusiastic clinician with an interest in Mental Health to join the team as a Senior Band 6 in a part time capacity of 30 hours per week.
Key responsibilities
 * Provide high quality mental health rehabilitation to patients referred to or within the Community Therapy Service (CTS).
 * Support other CTS practitioners to achieve rehabilitation goals.
 * Work closely with the Community Mental Health Team (CMHT) to minimise duplication and maximise efficiency.
 * Facilitate a seamless pathway between services for patients and establish care plans.
 * Assist with the community response physical health pathway.
Qualifications & experience
 * Senior clinician with a mental health background.
 * Enthusiastic to develop clinical practice within innovative and supportive multidisciplinary teams.
 * Experience treating patients in their own homes.
