IGPc Primary Care Network is looking for an enthusiastic Care Coordinator to join our forward-thinking Primary Care Network team.
The care coordinator role will work closely with other personalised care teams within the PCN to help and support multi-disciplinary team (MDT) working and ongoing patient case management. This will involve linking with the PCN's member practices and a range of external partners including Community Health Services, Voluntary and Community Sector, and the local authority.
This role will be key in supporting staff within iGPc to:
* Deliver effective, coordinated care for vulnerable and frail patients, particularly those at high risk of hospital emergency admission, emergency department attendances, or out-of-hours care.
* Assist with the coordination of Network clinics and attend community events where appropriate, promoting a neighbourhood working approach.
* Support the PCN member practices with specific tasks.
* Liaise with additional roles staff in the PCN to meet unmet patient needs.
Main duties of the job
Key Responsibilities
Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care.
Holistically bring together all of a person's identified care and support needs, exploring options to meet these within a single personalised care and support plan.
Help people manage their needs by answering queries, managing appointments, and providing good quality information to support informed choices about their care.
Support individuals to understand their health and wellbeing, including through the use of the Patient Activation Measure.
Assist access to self-management education, peer support, or interventions to support health and wellbeing and increase activation levels.
Provide coordination and navigation for people and their carers across health and care services, working with social prescribing link workers, health and wellbeing coaches, and other primary care professionals.
Support the coordination and delivery of MDTs within the PCN and contribute to effective MDT meetings.
Work with GPs and other primary care professionals to identify and manage a caseload of patients, referring as appropriate.

Job responsibilities
As previously outlined, with emphasis on proactive patient identification, holistic care planning, and effective MDT collaboration. Additional responsibilities include administrative support, case management, and supporting shared decision-making and anticipatory care planning.
Person Specification
Experience in patient-focused environments, planning, and organising meetings, supporting service improvements, and familiarity with EMIS software.
Qualifications include NVQ Level 3 or equivalent, with evidence of ongoing professional development.
Skills include IT proficiency, data analysis, excellent communication, interpersonal skills, kindness, compassion, teamwork, initiative, adaptability, persistence, and willingness to travel across practices.
Other requirements
This role is subject to a DBS check and involves travel between sites, with a valid driving license advantageous.
Salary: £27,485 per year, in line with the 25/26 uplift.
