30 hours per week. An exciting opportunity has arisen to be part of the Integrated Community Stroke Rehabilitation Team in Barnsley.


Job Summary

You will work as part of a multi‑disciplinary team of nursing and allied health professionals in both community and inpatient settings. Your duties will be carried out in a competent manner, requiring initiative, effective communication, and strong time management skills. A vehicle is essential for travel in connection with the post.


Job Responsibilities

* Work as part of a multi‑professional team in inpatient and community settings, providing safe and compassionate care that maximises patient recovery and rehabilitation potential.
* Carry out rehabilitation programmes set by therapists and the nursing team.
* Assist clinical staff with patient observations on the inpatient unit.
* Use excellent communication to support and motivate patients to participate in their rehabilitation.
* Participate in training, education, and clinical supervision to develop skills and knowledge.
* Work independently and as part of a team in the community.
* Implement policies, procedures, and protocols as per established clinical care plans.
* Use equipment such as hoists and wheelchairs to assist moving and handling of service users, ensuring proper instruction has been received.
* Deal with verbal and physical aggression using de‑escalation techniques; may involve restraint or seclusion when appropriate.
* Attend to the personal care needs of service users as required.
* Participate in a flexible rostering arrangement providing 24‑hour/7‑day care where required.
* Encourage up‑to‑date flu vaccination for staff to protect patients.

* Responsive attitude and approach.
* Dress appropriately for the environment.
* A current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for applicants who are unable to drive due to a disability).


Special Knowledge & Skills (Essential)

* Excellent customer care skills.
* Enthusiastic and able to work flexibly on a 24‑hour shift pattern to meet service user needs.
* Safe decision‑making skills commensurate with the role.
* Excellent communication and IT/keyboard skills.
* Diversity and cultural understanding.
* Knowledge of confidentiality and trust responsibilities.
* Resilience to pressure and exposure to emotional or hazardous situations.
* Non‑judgemental, empathetic manner.
* Demonstrates Trust values.
* Excellent interpersonal and organisational skills.
* Use of observation skills to identify potential risks and act appropriately.
* Ability to fulfil the value‑based induction.


Special Knowledge & Skills (Desirable)

* Language skills other than English.
* Experience supervising or inducting others.


Physical Attributes (Essential)

* A satisfactory sickness record over previous two years (subject to fairness and equality of opportunity).
* Ability to fulfil occupational health requirements for the post, with reasonable adjustments if necessary.


Training (Essential)

* Willingness to undertake all trust mandatory training and relevant refresher training.
* Willingness to undergo job development and training.
* Demonstrate excellent knowledge of service user safety, including prevention and management of violence and aggression, moving and handling.


Experience (Essential)

* Providing personal care and related procedures for individuals with health/social needs.
* Working in a caring environment at a Band 2 or equivalent level of competence.
* Dealing with the public and working as an effective team member.
* Demonstrating initiative, decision‑making and prioritisation skills.
* Commitment to user/carer involvement.


Qualifications (Essential)

* GCSE A‑D or grade 3–9 in Maths and English or equivalent.
* Level 3 apprenticeship in Health & Social Care or equivalent NVQ Level 3.
* Or, a level 2 qualification with demonstration of competencies at level 3.
* Care Certificate or requirement to achieve within 12 weeks of commencement.


Qualifications (Desirable)

* Level 4 units in Health & Social Care.
* Experience in the speciality area.
* Willingness to undertake further study towards a level 4 qualification.
