Band 6 Older Adult Specialist – Occupational Therapist or Registered Mental Health Nurse – permanent contract – 37.5 hours per week – site: St Andrews House, Town Derby – salary £39,959 – £48,117 per annum (pro rata).

We are looking for a specialist who can support older adults in the community with functional mental health conditions, help introduce them into the Living Well Team, identify needs, design person‑centred support and collaborate with local agencies.
Main duties of the job
* Clinically responsible for supporting the delivery of a flexible, responsive local service.
* Take forward an Integrated Wellbeing Pathway in the designated locality.
* Employ a wide range of skills, build therapeutic relationships and develop care plans.
* Conduct comprehensive person‑centred psychosocial and risk assessments.
* Support and motivate individuals to improve mental health, build resilience and utilise community resources.
* Maintain relationships with community partners, statutory, VCSE organisations and local communities.
* Respond to the voice of lived experience to develop service.
Qualifications
* Relevant qualification (Diploma in Nursing, Degree in Mental Health Nursing, or Degree in Occupational Therapy).
* Further training relevant to the role.
Experience
* Post‑qualification experience with completion of preceptorship or equivalent.
* Experience working with older people with functional mental health diagnoses.
* Experience of community working.
* Able to meet the travel requirements.
* Able to work independently and part of a team.
* Experience linking with partner agencies.
* Understanding of the Living Well programme.
* Good time management.
