Job Overview
As a Mental Health Community Case Worker, you will support adults to live as independently and safely as possible. You’ll receive referrals, undertake assessments, develop outcome‑focused care plans that promote choice, control, and wellbeing, manage a varied caseload, maintain accurate records, coordinate cost‑effective care packages, identify and respond to safeguarding concerns, contribute to legal processes where required, and lead case reviews and planning meetings.
Location
One role will be located in the Crisis Resolution and Home Treatment Team and the other in the Dementia and Older People’s Team in Longford.
Responsibilities
* Receive referrals and undertake assessments.
* Develop outcome‑focused care plans promoting choice, control, and wellbeing.
* Manage a varied caseload and maintain accurate records.
* Coordinate cost‑effective care packages tailored to individual needs.
* Identify and respond to safeguarding concerns.
* Contribute to legal processes where required.
* Arrange and lead case reviews and planning meetings.
Qualifications and Requirements
* Organised, proactive, and committed to delivering high‑quality support to vulnerable adults.
* Strong communication and assessment skills.
* Ability to manage a busy caseload and make informed decisions in line with policy and legislation.
* Confidence working both independently and as part of a multi‑agency team.
* Good understanding of safeguarding, risk management, and person‑centred care.
* Willingness to learn and stay up to date with relevant legislation, services, and best practice.
* Passionate about helping people maintain independence, dignity, and quality of life.
Administrative Requirements
* Post is exempted under the Rehabilitation of Offenders Act 1974 and will require a satisfactory DBS check.
* All candidates must have the Right to Work in the UK.
