Overview
Northumbria Primary Care (NPC) is seeking a motivated practice nurse to join our team at the Base site, NPC 49 Marine Avenue. The role is currently part‑time, with the potential to increase hours or take part of a job share.
Responsibilities
* Provide high standard enhanced nursing services to the entire practice population, including chronic disease management, health promotion, and maintaining immunisation and screening programmes.
* Conduct long‑term condition reviews (Asthma, Hypertension, COPD, Diabetes, etc.), smears, baby immunisations, blood sampling, and travel vaccines.
* Act as a role model of compassionate and inclusive leadership, engaging, enabling, and empowering others through coaching and facilitating teamwork across teams, departments and other GP practices within NPC.
* Collaborate with the current Practice Nurse and Healthcare Assistants, and work unsupervised when required, demonstrating critical thinking and decision‑making skills in patient management.
* Participate in training and educational programmes for staff and students as required.
* Possible occasional work across other GP practices within the NPC group, maintaining continuity of care.
Qualifications
Applicants should have primary care experience and be fully competent in primary care nursing duties, demonstrating consistently high standards of clinical excellence.
