About the Role

The Adur Neighbourhood Mental Health Team are looking for a qualified and experienced Mental Health Practitioner – Mental Health Nurse, Occupational Therapist or Social Worker – to work in a supportive multi‑disciplinary setting. The role involves caring for patients experiencing mental health difficulties on a community basis.


Responsibilities

* Maintain patient safety in collaboration with patients, carers and other team members.
* Recognise potential signs of patient harm, abuse or neglect and take all reasonable steps to protect the patient.
* Provide clinical advice on complex issues to other members of the clinical team and staff from other disciplines.
* Undertake urgent and priority assessments, including risk assessments.
* Make decisions and coordinate care in crisis situations.
* Manage complex patients who present to the team when a Care Coordinator/Lead Professional is absent and urgent response is required.
* Support the team where Intensive Home‑Based Treatment is required.
* Liaise with other services such as Police and A&E as required.
* Participate in setting quality standards and offer a consistent approach to working within the team.
* Support the team to deliver a range of interventions in accordance with the agreed plan of care.
* Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
* Assist team members with specialist assessments, e.g. application of the Mental Capacity Act 2005, Deprivation of Liberty Safeguards, Care Act s42 regarding Safeguarding Adults and personal safeguarding measures.
* Maintain professional standards of care at all times.
* Communicate effectively with all stakeholders – service users, carers, third‑sector providers, internal and external agencies and services.


Qualifications

* Registered Nurse or health professional with current registration appropriate to the role.
* Completion of a Mentorship Course/ENB or professional equivalent.
* Evidence of continuous professional development and significant post‑qualifying experience in the relevant care group (mental health or neurodevelopmental).
* Experience working alongside patients with complex health problems in the relevant care group.
* Experience supervising staff.
* Experience providing a range of clinical interventions to patients in the relevant care group with varied health problems.
* Experience working consultatively with other professionals.
* Experience conducting clinical assessments including risk assessments.
* Experience partnership working within and across statutory and non‑statutory teams and agencies.
* Experience working within the CPA process.
* Understanding of clinical governance.
* Previous experience working in the community.
* Band 5 or newly qualified nurse with previous mental health experience who wishes to develop into a community mental health nurse role.
* Band 5 or above technical qualifications or equivalent experience in occupational therapy or social work.
* UK professional registration.
* Ability to travel independently across the locality and have access to a suitable vehicle for business purposes.


Working Conditions

Usual hours are Monday to Friday 9am – 5pm. The role requires independent travel across the locality.
