About the role

We are looking for a high-performing and experienced Adults’ Social Worker to join our innovative and forward-thinking local authority. We currently have a vacancy in our Preparation for Adulthood & Autism Team, which supports young people transitioning into adult services and manages autism referrals. This dynamic role offers the opportunity to work closely with:

* The Transition Service
* Children with Disabilities Team
* Leaving Care Team
* Special Educational Needs (SEN) services
Key Responsibilities
* Carry out timely and responsive assessments, re-assessments, and reviews to deliver personalised care and support to service users and carers.
* Promote independence, safety, and wellbeing through person-centred planning.
* Case management, EHCPs, Health Action Planning, and duty management within a fast-paced environment.
* Manage safeguarding referrals under Section 42 and Community Deprivation of Liberty Safeguards (DoLS).
* Undertake Mental Capacity Act assessments, Best Interest decisions, and Community DoLS applications.
* Participate in Continuing Health Care (CHC) assessments.
* Support young people in accessing employment opportunities.
* Liaise effectively with statutory bodies, voluntary organisations, and other partner agencies.
* Make referrals to supported living services and develop risk assessments and protection plans.
* Contribute to service development, represent the team when required, and share best practices across the service.
What We’re Looking For
* An experienced Adults’ Social Worker with a strong understanding of transition and autism pathways.
* A proactive and organised individual with a commitment to improving outcomes for young people.
* Strong knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding protocols.
