Nurse Associate - Parkgate Medical Practice

The post holder is responsible for thedelivery of nursing care to the practice population. Supported by practice nurses,they will deliver care within the boundaries of their role, focusing upon supportingpatients to be healthy, monitoring of long-term conditions, health preventionand screening activities. They will work collaboratively with the generalpractice team to meet the needs of patients, supporting the delivery of policyand procedures, and providing nurse associate leadership as required.


Main duties of the job

ClinicalPractice

* Promote health andwell-being, and prevent adverse effects on health and well-being
* Implement and evaluateindividual treatment plans for patients with a known long-term condition
* Identify, and manage asappropriate, treatment plans for patients at risk of developing along-term condition
* Prioritise health problemsand intervene appropriately to assist the patient in complex, urgent oremergency situations, including initiation of effective emergency care
* Work with patients in order to support adherenceto prescribed treatments
* Support patients to adopthealth promotion strategies that encourage patients to live healthily, andapply principles of self-care
* Deliver opportunistichealth promotion using opportunities such as new-patient medicals

We operateNHS services across the North East, North West and Yorkshire.


Job responsibilities

Clinical skills

* BP, height, weight,urinalysis, smoking cessation, lifestyle advice
* Cervical screening
* Chronic disease managementreviews including asthma, diabetes, COPD, learning disability.
* Venepuncture
* ECG
* Complex wound care
* To be able to administerIM injections including NHS selective programmes, NHS additional vaccinationsand NHS supported travel vaccinations which may include- shingles, pneumococcal,influenza, Hepatitis A, Hepatitis B, Typhoid, Boostrix, Meningitis, ACWY,HiB, and Revaxis under a Patient Specific Directive.
* Oral contraception checks andrefer to a practice nurse or GP when appropriate
* To administer and trainpatients to self administer injectable contraception under a Patient SpecificDirective such as Depo-Provera and Sayana Press
* To administer mentalhealth depo injections under a Patient Specific Directive.
* Suture and staple removal
* Prepare specimens forcollection by the pathology laboratory.
* Insert Zoladex implants

Communication

* Utilise and demonstrate sensitivecommunication styles, to ensure patients are fully informed and consent totreatment
* Communicate effectivelywith patients and carers, recognising the need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackground and preferred ways of communicating
* Utilise communicationskills to support patients to adhere to prescribed treatment regimens
* Anticipate barriers to communicationand take action to improve communication
* Act as an advocate whenrepresenting the patients and colleagues viewpoints to others
* Recognise and work within theNurse Associate competence and professional code of conduct as regulatedby the Nursing and Midwifery Council (NMC)
* Produce accurate,contemporaneous and complete records of patient consultation, consistentwith legislation, policies and procedures
* Prioritise, organise andmanage own workload in a manner that maintains and promotes quality
* Deliver care according tothe NSF and the National Institute for Clinical Excellence (NICE) guidelinesand evidence-based care
* Participate in the maintenanceof quality governance systems and processes across the organisation andits activities
* In partnership with otherclinical teams, collaborate on improving the quality of health care, respondingto local and national policies and initiatives as appropriate
* Support and participate inshared learning across the practice and wider organisation
* Participate in theperformance monitoring review of the team, providing feedback as appropriate
* Work within policiesrelating to domestic violence, vulnerable adults, substance abuse andaddictive behaviour, and refer as appropriate safeguarding children

* Act as mentor to students,assessing competence against set standards as requested and ifappropriately qualified
* Disseminate learning andinformation gained to other team members in order to share good practiceand inform others about current and future developments

ManagingInformation

* Use technology as an aidto management in planning, implementation and monitoring, presenting andcommunicating information
* Review and process datausing accurate Read codes about patients in order to ensure easy andaccurate retrieval for monitoring and audit processes
* Manage informationsearches using the internet Understand own and others responsibility tothe individual organisation regarding the Freedom of Information Act

Person Specification


Qualifications

* Registered Nurse Associate
* Current NMC registration and revalidation with NMC
* Evidence of continuous professional development.
* Work without direct supervision.
* Qualifications/experience in Cervical cytology.
* Immunisation and Vaccine training
* Chronic Disease Management experience in 1 or more chronic disease.


Experience

* Experience of treatment room duties required for the post.
* Awareness of clinical governance issues in primary care.
* Knowledge of patient specific directives and associated policies.
* Experience of quality initiatives such as QOF and KPIs
* At least 1 years experience of working in general practice
* Knowledge of local health issues.
* Awareness of local and national health policy

* Qualification / Experience in running chronic disease management clinics.
* Demonstrate an understanding of the data protection act and patient confidentiality, clinical and information governance and Caldicott requirements.
