Description Supported Assistant Practitioner/Assistant Practitioner 8862| Fixed term contract until 21 September 2026 | £28,598 to £33,699 per annum (Scale F/G) | 37 hours per week | Greyfriars House, Great Yarmouth An exciting opportunity has arisen for an enthusiastic Assistant Practitioner as a secondment opportunity to join the Eastern Home First Team, within the Eastern Locality to provide timely follow up assessment to those people discharged from acute hospitals under Discharge to Assess (D2A) and the NHSE Hospital Discharge Service: Policy & Operating Model. Norfolk County Council are on an ambitious journey to ensure adults being discharged from the James Paget University Hospital, other acute hospitals, and community short term placements, are supported to return home, access reablement opportunities and plan their ongoing care and support needs once discharged. The Homefirst Team are working towards an integrated care system, alongside Health; Integrated Care Board, Eastern Locality Primary Care Hubs, Norfolk First Support and Caring for Better Outcomes providers. The team provides in-reach support with the Acute Hospital, as well as following up transfers of care within the Virtual Homefirst Hub, providing Care Act Assessments, Carers assessments and reviews. With our integrated working, you’ll enjoy variety in your day and the opportunity to build strong working relationships with professionals across the county council, NHS and other partners. Utilising our Living Well model, changes the conversation for social work to provide better outcomes and aligns with our vision to support people to be independent, resilient and well. Talking about what they can do, listening and connecting to enable early intervention in the community and avoid where possible admission to an acute setting. You’ll manage your own diary with the autonomy to decide on face to face visits, and we offer flexible remote working with office access and homeworking as your base.
